Project Objectives:
This project integrates artificial intelligence (AI) into medication reconciliation to identify patients with diabetes, ensuring more accurate and targeted interventions.
Target Population:
Patients with diabetes receiving care at UAB Medical Center or Cooper Green Mercy Health Services—particularly those managing complex medication regimens or affected by non-clinical determinants of health during post-discharge care transitions.
Relevance to Community:
Medication discrepancies contribute to poor diabetes outcomes, particularly among patients with health literacy challenges or financial constraints. This project addresses these gaps by optimizing resource allocation through AI-guided risk stratification.
Intervention/Approach
Strategies Used:
Develop machine learning models to identify patients with complex medical history for medication discrepancies.
Incorporate non-clinical determinants of health data into risk stratification.
Pilot the AI system within routine hospital workflows.
Relevance:
The project acknowledges differences in medication access and adherence, designing the AI system to incorporate economic and health literacy considerations.

Recruitment Details
Participation Information:
The study is conducting a mixed methods study to develop an AI system for patients with diabetes at UAB Medical Center and Cooper Green Mercy Health Services.
Eligibility Criteria:
Participants must have a diabetes diagnosis and be at risk for medication discrepancies with recent (< 6 months) hospitalization or ED visit.
